Job Summary
Director leads the clinical Case Management team, coordinating case managers and social workers, developing and implementing case management processes, promoting high-quality patient outcomes, and supporting utilization/resource management. Responsibilities include supervising staff, promoting quality patient care, coordinating multidisciplinary processes, and identifying opportunities to improve patient care and services. Responsibilities emphasize leadership, collaboration with physicians and leaders, and development of tools and systems for effective case management. Required Qualifications
- Registered Nurse (RN) with acute hospital nursing experience required
- Bachelor Degree required
- Director of Case Management in an Acute Hospital Setting required
- Bachelor's or Master’s in Nursing (BSN/MSN) preferred
- Certification in Case Management (CCM/ACM) preferred
