Director of Care Management Social Work II

Directs and oversees the Care Management department in an acute hospital setting, driving care coordination, discharge planning, utilization management, and transitions of care; leads staffing, and collaborates with physicians and executives to optimize patient outcomes, reduce readmissions, and ensure regulatory compliance. Demonstrates leadership, change management, and staff development to align with CMS requirements and organizational goals. Mentors facility Care Management staff and interdisciplinary teams on data, outcomes, and opportunities; actively engages with post-acute care entities to improve patient care transitions.
