Job Summary
Direct and oversee care management, discharge planning, and utilization review across two acute care hospitals in the Pacific Northwest. Lead an interdisciplinary team of nurses and care management professionals, partner with physicians, community entities, and payors to ensure safe, coordinated transitions of care, and drive systemwide initiatives to improve quality, efficiency, and patient outcomes using data analytics.
Required Qualifications
- Bachelor’s degree in Nursing (RN)
- RN with at least 2 years of acute care experience
- 3+ years in care coordination or utilization review leadership at the Director level
