Job Summary
Provide primary care for an assigned patient panel, managing acute and chronic conditions while supporting preventive care and wellness visits. Collaborate with nursing, APPs, and interdisciplinary staff to lead clinical teams and mentor providers. Participate in care planning, population health initiatives, and quality metrics to ensure continuity of care. Deliver same-day, next-day, and urgent visits as needed while covering rotating call shifts. Full-time outpatient role in Dayton, Ohio, Monday through Friday from 8:00 AM to 5:00 PM.
Required Qualifications
- MD or DO
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Primary Care-related specialty
- Active Ohio medical license or ability to obtain
- Primary care experience required
- Monday through Friday schedule
- 8:00 AM to 5:00 PM
- Provide primary care for an assigned patient panel
- Manage acute and chronic medical conditions
- Support preventive care, wellness visits, and follow-up care
- Work closely with nursing, APPs, care management, pharmacy, behavioral health, and support staff
- Help lead and support the clinical team
- Mentor and collaborate with physicians and Advanced Practice Providers
- Participate in care planning and population health initiatives
- Support quality metrics, patient outcomes, and continuity of care
- Provide same-day, next-day, and urgent visits as needed
- Participate in rotating call coverage as required
- Strong communication and team-based care skills
- Interest in value-based care and population health
