Job Summary

The role emphasizes upstream education, preventative care to delay CKD progression, comprehensive patient support and resource navigation, and ensuring smooth transitions to dialysis when needed. Required Qualifications
- Bachelor’s degree (related area)
- Minimum of six years’ supervisory experience
- Intermediate computer skills in MS Word, Excel, PowerPoint, and Outlook
