Director, Enhanced Care Management

Job Summary
Director of Enhanced Care Management responsible for directing ECM program operations, CalAIM program performance, provider and community engagement, supervision of ECM staff, policy development, process improvements, and risk management. Leads development and execution of ECM strategy, oversees contract performance and regulatory/data requirements, manages records and billing, oversees assessments and care planning, staff training, and KPI development. Collaborates with leadership across departments, focuses on delivering high-quality patient care, cost-effective outcomes, and scalable program growth. Requires 5+ years of people management, 3+ years in care management, and experience with ECM programs; remote work allowed with occasional travel; open to applicants in multiple US states. Vaccination may be required for patient-facing roles.
Required Qualifications
- Bachelor's degree (highly preferred)
- 5+ years of people management
- 3+ years of care management experience
- Experience with Enhanced Care Management programs preferred
- P&L responsibility experience
- Strong organizational, communication, and cross-functional collaboration skills
- Ability to work in a remote environment
- Access to reliable internet
- Experience with diverse populations and multidisciplinary teams
