Job Summary
Director of Case Management responsible for overseeing hospital case management operations, ensuring staff compliance with organizational policies and external regulatory agencies, and coordinating the integration of discharge planning and patient-care processes with related hospital departments and external agencies to ensure continuity of care and optimal clinical resource utilization. The Director has oversight for daily case management operations, leads interdisciplinary teams and external stakeholders, and must possess executive presence with strong communication and presentation skills. Requires RN licensure or LCSW and a Bachelor's degree, with 5-7 years of acute care case management experience and 2+ years in leadership. This role emphasizes leadership, change management, and the ability to manage multiple priorities with minimal supervision.
Required Qualifications

- Either Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW), required
- 5-7 years’ experience in overall acute care Hospital Case Management
- 2+ years’ experience in case management leadership
