Director Case Management

Directs and oversees a team of clinical professionals in Case Management, Care Coordination, and Care Transitions within an acute hospital setting. Leads development and implementation of Case Management processes, supervises clinical Case Managers and Social Workers, promotes high-quality patient outcomes, and supports utilization and resource management. Facilitates multidisciplinary collaboration, identifies opportunities to improve patient care and services, and executes collaborative practices aligned with patient-centered care and community engagement.
