Job Summary
Lead system-wide care transformation and interoperability efforts by planning, developing, and administering public health programs focused on data collection, patient-centered medical homes, health IT, and emergency preparedness. Oversee program establishment, monitor performance against goals, and manage contracts, fund allocation, and compliance. Develop systems to analyze demographics and trends across primary care practices while collaborating with interagency stakeholders to align engagement efforts. Mentor administrative and support staff, ensuring high-quality service delivery and fostering professional development through consistent training plans. Build partnerships with Federally Qualified Health Centers and hospital-affiliated ambulatory centers to advance health information exchange and care coordination.
Required Qualifications
- A baccalaureate degree from an accredited college
- Five years of full-time professional satisfactory experience acquired within the last fifteen years in a health services setting
- At least 18 months of managerial experience
- Managerial experience demonstrating the ability to perform difficult and responsible managerial work, requiring independent decision-making concerning program management, planning, allocation of resources, and the scheduling and assignment of work
- A minimum of a baccalaureate degree
- 18 months of managerial experience as described in the primary requirement
Desired Qualifications
- Experience in a health services setting (e.g., laboratory, hospital, patient care facility, public health, environmental health, or mental hygiene program)
- Experience serving as a practice facilitator, quality improvement advisor, technical assistance provider, or program manager supporting primary care practices
- Knowledge of NCQA Patient-Centered Medical Home standards, quality measurement, clinical workflow redesign, care coordination, population health management, and performance improvement methodologies
- Experience leading or supporting value-based payment initiatives, alternative payment models, quality incentive programs, or health care delivery system transformation efforts
- Experience working with federally qualified health centers (FQHCs), hospital-a filiated ambulatory care networks, independent primary care practices, or community-based provider organizations
- Familiarity with quality improvement methodologies such as Plan-Do-Study-Act (PDSA), Lean, or Six Sigma
- Possession of a Patient-Centered Medical Home Certified Content Expert (PCMH CCE) credential
- Experience managing contracts, grants, technical assistance programs, or provider engagement initiatives in a public health or health care setting
- Experience analyzing quality, utilization, operational, or population health data to inform program planning and performance improvement efforts
- Proficiency in Microsoft Office Suite
- Experience with CRM systems (e.g., Salesforce)
