Job Summary
Provide direct support and personal care, including hygiene, bathing, grooming, toileting, oral care, and dressing. Promote independence by teaching self-help and social skills while administering medications if certified. Implement support plans and document progress in the Electronic Health Record. Ensure safety and well-being by reporting concerns of abuse, neglect, or mistreatment immediately. Drive agency vehicles to transport individuals safely and maintain vehicle logs. Support individuals in community activities, social relationships, and meaningful experiences. Complete required training and maintain certifications. Provide coverage at alternate program sites as needed. Work Monday through Friday from 8am to 4pm at a location in Rochester, NY.
Required Qualifications
- A compassionate, dependable, and caring nature
- Ability to read and follow service plans and safety protocols
- Basic computer skills for documentation in EHR
- Effective written and verbal communication skills
- Ability to problem-solve and adapt to different situations
- Valid Driver's License
- At least 2 years of driving experience
- Must meet Lifetime Assistance's Vehicle Operator Requirements
- Must successfully pass all required background checks and clearances
- Physical ability to assist individuals, including lifting/moving up to 35 pounds
- Physical ability to stand
- Physical ability to walk
- Physical ability to bend
- Physical ability to kneel
- Physical ability to perform occasional extended driving
- Monday - Friday 8am - 4pm availability
