Job Summary
Direct Support Professional/Van Driver split shift part-time role in Troutman, NC. Schedule: Monday-Friday 6:15am-9:00am and 2:00pm-4:30pm; shift can be split. Responsibilities include providing 1:1 care to individuals with disabilities in home and community settings, teaching skills to promote independence, supporting community inclusion, mentoring social skills, assisting with activities of daily living (eating, bathing, dressing, personal hygiene), meal preparation, health and safety monitoring, documenting activities in an electronic health record, and submitting payroll/documentation on schedule. May provide transportation to and from designated sites and assist with mobility needs using transfer techniques, a Hoyer lift and/or gait belt. Training is provided; must be at least 18 years old, have a driver’s license for 2+ years, pass background checks, and hold First Aid/CPR certification. LIFESPAN is an equal opportunity employer.
Required Qualifications
- High school graduate or equivalent (GED)
- Possess a valid driver’s license for a minimum of two consecutive years
- Pass criminal record and healthcare registry check
- Pass driving record check if providing transportation
- Note: Current First Aid/CPR blended or classroom certification is required
- Ability to lift up to 40 pounds independently (per policy)
