Job Summary

Conduct daily Habilitation services and behavioral support plans while documenting data collection and verifying compliance with NY OPWDD and Medicare/Medicaid protocols. Assist individuals with personal care, community activities, and transportation via agency vehicles, maintaining a safe, sanitized environment and adhering to strict infection control standards. Administer prescribed medications after completing the AMAP course and remain alert throughout assigned shifts, intervening promptly to address any incidents. Report all occurrences timely and participate in required staff meetings and training. Drive agency vehicles and operate office equipment while communicating fluently in English. Maintain a professional demeanor with residents, families, and co-workers, ensuring all regulations and the DSP Code of Ethics are followed.

Required Qualifications

  • High School diploma or equivalent
  • A valid driver's license
  • Completion of State and Agency mandated training
  • Communicate effectively orally and in writing
  • Represent yourself and the Agency in a professional and courteous manner
  • Know, understand, and continually practice the Agency's Standards of Conduct
  • Must be able to use a computer and learn software programs
  • Must be able to speak, read and write fluently in the English language
  • Must be able to write in the English language using correct spelling, grammar, punctuation, etc.
  • Must have a valid Driver's License
  • Must be willing to drive agency vehicles
  • Must be able to lift 0-25 lbs. (frequently)
  • Must be able to lift 25-50 lbs. (occasionally)
  • Must be able to lift over 50 lbs. (rarely)
  • Must be able to push/pull 0-25 lbs. of force (frequently)
  • Must be able to push/pull 26-45 lbs. force (occasionally)
  • Must be able to push/pull over 45 lbs. (rarely)
  • Must be able to sit 50-60% of the working day
  • Must be able to stand 10-20% of the working day
  • Must be able to walk 10-20% of the working day
  • Must be able to occasionally stoop
  • Must be able to occasionally kneel
  • Must be able to occasionally crouch
  • Must remain awake and alert during the entirety of the assigned shift
  • Must be able to operate a fax, copier, scan, and other office equipment
  • Must be able to administer prescribed medications after successful completion of AMAP medication administration course
  • Must complete AMAP medication administration course certification prior to the end of employee's probation period
  • Must maintain medication administration certification in accordance to outlined protocols
  • Must be able to drive agency vehicles for transportation of persons receiving supports
  • Must adhere to all related vehicle operation policies
  • Must adhere to all regulations, policies, procedures and protocols set forth by NY OPWDD, NY Justice Center, Centers for Medicare/Medicaid and Abbott House
  • Must maintain a safe and clean environment by adhering to sanitary and infection control procedures
  • Must attend and successfully complete all required training in accordance with SPDD Training Policy
  • Must be able to intervene and address any problem that may be occurring
  • Must report all incidents in a timely manner as required
  • Must participate in scheduled program meetings, i.e., staff meetings, in-service trainings and other meetings as required
  • Must provide support/coverage to other than assigned area as needed
  • Must perform all necessary activities assigned to further the agency's mission

Desired Qualifications

  • One-year experience working with people with developmental disabilities
