Job Summary
Supervise program participants at the program site and community locations, ensuring safety while assisting with activities of daily living including transferring, toileting, and ambulating. Lead planned small group activities and drive agency vehicles to facilitate community-based outings. Implement each participant's therapeutic and behavioral plans, review individual care plans annually, and provide information to Program Specialists to develop appropriate experiences. Maintain confidentiality, complete required documentation, and assist in developing instructional materials while adhering to agency policies and procedures. Operate wheelchair lifts and ramps safely and keep program equipment and areas clean and orderly.
Required Qualifications
- High School Diploma
- One year of experience providing services to individuals with disabilities
- Knowledge of community resources and means of accessing these resources
- Valid NYS driver's license
- Demonstrates basic reasoning and writing skills
- Must be able to lift and/or move objects up to 10 pounds
- Must be able to occasionally lift and/or move objects up to 30 pounds
- Must be able to regularly talk or hear
- Must be able to frequently sit, stand, walk, use hands to finger, handle or feel, and reach with hands and arms
- Must be able to regularly transfer individuals
- Must be able to regularly toileting, changing, ambulating and feeding individuals
- Must be able to recognize safety hazards
- Must be able to drive the agency vehicles
- Must be able to operate wheelchair lifts and ramps safely
- Must be able to maintain a safe environment at the program site and in the community
- Must be able to lead planned small group activities
- Must be able to supervise program participants at the program site and community locations
- Must be able to complete required documentation about individual's performance
- Must be able to access and maintain passwords and log ins for electronic health records, agency email, virtual trainings, and electronic employee records
- Must be able to adhere to agency's policies and procedures
- Must be able to participate in conferences, workshops, department meetings, in-service and on-line training
- Must be able to maintain confidentiality of information relating to individuals and their families
- Must be able to impart agency philosophy of Equal Opportunity, Independence and Realization of Individual Potential
- Must be able to function as a positive, contributing member of the TBI/NHTD/OPWDD program team
- Must be able to perform other duties at the discretion of the Program Supervisor and Director
