Job Summary
Support individuals with disabilities and mental health needs in their homes by assisting with daily living tasks such as transportation, bathing, meal preparation, and medical appointments. Collaborate with clients to achieve personal goals including healthy eating, exercise, and community activities like shopping or special events. Provide encouragement and coaching to help develop daily living skills while documenting all support provided during each shift. Work across a variety of needs including medical, behavioral, and mental health supports. Training and classes are provided to ensure excellent service delivery.
