Support clients with daily activities including grocery shopping, hygiene, and medication administration. Assist with mobility needs such as two-person transfers and Hoyer lift usage. Help individuals achieve personal goals like cooking, cleaning, and social integration by transporting them to appointments and community events. Document progress on Person Centered Service Plans and communicate regularly with supervisors. Maintain a valid driver's license and clean driving record, with flexibility to work varied shifts including weekends.
