Job Summary
Provide training and assistance to residents to develop independent living skills in cooking, housekeeping, laundry, and grocery shopping. Assist with personal care needs including bathing, toileting, and grooming while ensuring safety and well-being. Administer medications and complete required documentation such as progress notes and habilitation plans. Transport residents to medical appointments and community outings, maintaining cleanliness of the residence. Participate in service plan implementation, team meetings, and assessments. Act as a positive role model for household, social, and community skills. Follow agency policies, OPWDD regulations, and incident reporting procedures. Full-time schedule with 2nd and 3rd shifts; pay $20.00–$21.75/hour. High School Diploma and valid NY driver's license required.
Required Qualifications
- High School Diploma or GED
- Valid NY State driver's license with a driving record acceptable to the agency's insurance carrier
- Ability to lift and assist with up to 50 pounds (including transfers and physical support)
