Job Summary
Support individuals with disabilities in daily routines, personal care, and community integration while empowering them to lead self-directed lives. Accompany supported persons to events, job sites, and medical appointments using agency or personal vehicles, and assist with cooking, cleaning, and laundry as desired. Document shift notes in electronic records, manage medication storage, and ensure safe transport and resource utilization. Maintain confidentiality, attend training meetings, and consistently work toward Individual Support Plan outcomes. This role requires a valid driver's license, CPR/First Aid, and CPI certification, with a part-time schedule available from 7a-7p.
Required Qualifications
- 18 years of age
- able to read and write effectively
- high school diploma or GED
- valid drivers license
- acceptable driving record
- meet state and local requirements regarding criminal background check
- meet state and local requirements regarding sexual offender check
- meet state and local requirements regarding abuse registry
- meet state and local requirements regarding drug and felony offender listings
- meet all training requirements
- keep training current
- TB Skin tests
- HEP B vaccinations
- Flu vaccinations
- full range of body movements including the use of his/her hands, feet, and limbs
- ability to bend the body, to reach for objects, and to crouch/stoop
- ability to climb when needed
- command of all five senses: sight, hearing, touch, smell, and taste
- close vision
- distant vision
- depth perception
- ability to talk and listen for prolonged periods both on the telephone and face to face
- ability to perform heavy lifting over 50 lbs
- Valid state issued Driver License
- valid Medication Certification
- valid CPR/First Aid Certification
- valid CPI (Crisis Prevention Institute) training
