Job Summary
Implement individuals' plans consistent with their specific services, goals, and outcomes while ensuring physical and emotional well-being. Support and teach individuals through skills development, socialization, and task participation across all settings to promote community inclusion and foster friendships with neighbors and peers. Transport individuals or assist with public transportation, participate in recreational outings, and actively engage in daily activities such as toileting, meal prep, and behavioral interventions. Document necessary information in medical, behavioral, and communication books, complete written reports, and report incidents to management immediately. Safeguard confidentiality, ensure equipment storage, maintain cleanliness standards, and provide appropriate supervision. Maintain certifications in AMAP, CPR/First Aid, and SCIP, and meet Core Competencies Standards within the first year.
Desired Qualifications
- High School Diploma, GED or CNA (Certified Nursing Assistant)
- One year or more experience working with individuals with developmental disabilities
- Valid NYS driver's license
