Job Summary
Provide support to program participants to ensure a nurturing, safe, and healthful living environment, including personal hygiene, food preparation, medication monitoring, and community outings. Coordinate services consistent with the Individual Instruction and Support Plan while maintaining professional relationships with families and staff. Complete Per Diem checklists monthly, attend required training, and submit timely documentation including incident reports and shift notes. Maintain a valid driver's license to transport clients as needed.
Required Qualifications
- Must be at least 18 years of age or older
- Must have a valid driver's license and car
- Must be willing to transport clients in their own vehicle
- Must have a valid in-state driver's license
- Must be able to provide current car insurance
- Must pass a Washington State background check
- Must have all training before becoming a Per Diem employee
- Must keep up to date with all training
- Must complete Per Diem checklist and training on a monthly basis
- Must be familiar with the Residential Service Guidelines issued by the Division of Developmental Disabilities
- Must enroll and complete the Nurse Delegation study course and take tests within 30 days of employment
- Will become Nurse Assistant Registered (NAR) at employee's own expense initially and annually
- Must maintain active NAR
- Must maintain active CPR/First Aid
- Must maintain active Nurse Delegation
- Must maintain other required certifications
- Must complete a minimum of 12 hours of continuing education annually
- Must obtain CPI Blue Card
- Must be First Aid/CPR Certified (within 120 days of employment)
- Must be Blood Borne Pathogen certified (within 120 days of employment)
- Must be able to pick up a minimum of (1) shift per week or (2) shifts per pay period
- Must be available for weekend shifts
- Must be available for on-call shifts
- Must be available for open shifts
- Must be available to cover as needed
- Must be able to work up to 31 hours a week
- Must be willing to work multiple programs
- Must be willing to orient themselves to plans in the program before working with clients
- Must be willing to attend staff meetings as required
- Must be willing to attend training offered/required by the agency, DDD, and other parties
- Must be willing to treat clients with dignity and respect
- Must be willing to provide each participant with an array of instruction and support services
