Job Summary

Support persons receiving services while promoting their personal, physical, and emotional wellbeing alongside their goals. Implement outlined Habilitation and Behavioral Support plans, document daily data collection, and administer prescribed medications after completing the AMAP course. Assist with personal care, plan community activities, and drive agency vehicles for transportation. Maintain a safe, clean environment by adhering to sanitary protocols and infection control procedures. Intervene and address problems as they occur, report incidents timely, and ensure the residence is sanitized. Adhere to all NY OPWDD, Justice Center, and Medicare/Medicaid regulations and policies. Remain awake and alert throughout the assigned shift, attend required training, and provide coverage as needed. Work 40 hours weekly; additional time may be required until a relief staff is in place.

Required Qualifications

  • High School diploma or equivalent
  • A valid driver's license
  • Completion of State and Agency mandated training
  • Communicate effectively orally and in writing
  • Represent yourself and the Agency in a professional and courteous manner
  • Know, understand, and continually practice the Agency's Standards of Conduct
  • Must be able to use a computer and learn software programs
  • Must be able to speak, read and write fluently in the English language
  • Must be able to write in the English language using correct spelling, grammar, punctuation, etc.
  • Must have a valid Driver's License
  • Must be willing to drive agency vehicles
  • Must be able to lift 0-25 lbs. (frequently)
  • Must be able to occasionally lift 25-50 lbs.
  • Must be able to rarely lift over 50 lbs.
  • Must be able to frequently push/pull 0-25 lbs. of force
  • Must be able to occasionally push/pull 26-45 lbs. force
  • Must be able to rarely push/pull over 45 lbs.
  • Must be able to sit 50-60% of the working day
  • Must be able to stand 10-20% of the working day
  • Must be able to walk 10-20% of the working day
  • Must be able to occasionally stoop
  • Must be able to occasionally kneel
  • Must be able to occasionally crouch
  • Must be able to regularly stand

  • Must be able to operate a fax, copier, scan, and other office equipment
  • Must be able to administer prescribed medications after successful completion of AMAP medication administration course
  • Must be able to learn and demonstrate skills outlined in the DSP Code of Ethics and DSP Core Competencies
  • Must be able to attend and successfully complete all required training in accordance with SPDD Training Policy
  • Must be able to intervene and address any problem that may be occurring
  • Must be able to report all incidents in a timely manner as required
  • Must be able to drive agency vehicles for transportation of persons receiving supports and adhere to all related vehicle operation policies
  • Must be able to maintain a safe and clean environment by adhering to sanitary and infection control procedures
  • Must be able to establish positive working relationships with persons receiving services, co-workers, management and the public
  • Must be able to maintain a professional demeanor when interacting with parents, guardians and other professionals
  • Must be able to implement outlined Habilitation and Behavioral Support plans/programs and document accordingly
  • Must be able to assist with personal care of persons receiving support as needed
  • Must be able to assist individuals with planning and participating in the community activities
  • Must be able to ensure residence is cleaned and sanitized
  • Must be able to participate in scheduled program meetings, i.e., staff meetings, in-service trainings and other meetings as required
  • Must be able to provide support/coverage to other than assigned area as needed
  • Must be able to perform all necessary activities assigned to further the agency's mission
  • Must be able to adhere to all regulations, policies, procedures and protocols set forth by NY OPWDD, NY Justice Center, Centers for Medicare/Medicaid and Abbott House

Desired Qualifications

  • One-year experience working with people with developmental disabilities
