Job Summary
Direct Support Professional providing hands-on care, advocacy, and supports to individuals with developmental and behavioral health needs in home and community settings. Responsibilities include personal care, daily living support, medication administration as directed, health and wellness coordination, mobility assistance, behavioral and emotional support, and collaboration with healthcare teams. Documentation of daily notes, progress/incident reports in electronic systems, and active participation in community activities and planning. Requires high school diploma or equivalency and a NYS driver’s license; strong communication in English and a compassionate, empowering approach to help individuals achieve independence.
Required Qualifications
- High School Diploma or equivalency diploma (required)
- Fluent in English for client communication
- A valid New York State Driver’s License with at least 2 years of driving experience (required for driving positions)
- Comfort operating a large vehicle (for driving positions)
