Job Summary
Provide direct care and supervision to individuals served, ensuring their health, safety, and well-being. Implement Individual Service or Treatment Plans, Behavior Support Plans, and program routines. Assist with activities of daily living including personal care, meals, mobility, and household tasks. Facilitate activities promoting skill development, independence, recreation, and community engagement. Monitor and document progress, behaviors, health concerns, and incidents. Safely transport individuals to appointments, activities, or community outings. Maintain a clean, safe, and supportive environment while following all safety and regulatory requirements. Communicate effectively with team members, supervisors, and other stakeholders to support coordinated care.
