Job Summary
Provide daily support and care to individuals in a residential setting, helping them build independence through life skills training at home and in the community. Implement Individual Support Plans and Behavior Support Plans while assisting with personal care, medication administration, and health needs. Accompany individuals to appointments, support community activities and relationships, and assist with budgeting, shopping, and managing personal funds. Maintain accurate documentation, ensure a clean and safe home environment, and take a leadership role by training and mentoring other staff. Build strong relationships with families, guardians, and team members.
Required Qualifications
- High school diploma, GED, or HiSET
- Active MAP (Medication Administration Program) certification
- Valid driver's license
- reliable transportation
