Job Summary
Provide daily support and care to individuals in a residential setting, assisting with personal care, medication administration, and health-related needs. Help individuals build independence by teaching life skills at home and in the community while following Individual Support Plans and Behavior Support Plans. Accompany individuals to appointments, support community activities and social connections, and assist with budgeting, shopping, and managing personal funds. Maintain accurate documentation to track progress and outcomes. Take a leadership role by training and mentoring other staff, building strong relationships with families and team members to ensure a clean, safe environment.
Required Qualifications
- High school diploma, GED, or HiSET
- Active MAP (Medication Administration Program) certification
- Valid driver's license
- Reliable transportation
