Job Summary
Perform personal care tasks including assistance with hygiene, grooming, feeding, and ambulation while monitoring medical needs. Implement Individual Service Plans and Behavior Support Plans to manage behaviors through prevention and de-escalation. Encourage self-help activities and foster positive relationships between individuals served and their support circles. Maintain accurate daily documentation of activities, medications, and goal tracking. Transport individuals to community activities and medical appointments, or run errands as needed. Required Qualifications
- Eighteen years of age or older
- Pass background check
- Driver's license with valid Auto Insurance
- High school diploma/GED
- Direct Support Professional (DSP) certification; or Copy of high school diploma/transcript or General Education Development (GED) diploma; and at least six (6) months of experience providing behavioral health related services to individuals with Intellectual and Developmental Disabilities (I/DD), or documented experience providing specific supports to individuals with disabilities
