Job Summary
Perform personal care tasks including assistance with basic hygiene, grooming, feeding, and ambulation while monitoring medical needs. Implement Individual Service Plans and Behavior Support Plans to manage client behaviors through prevention or de-escalation of aggression. Encourage self-help activities and foster positive relationships between individuals served and their support circles. Maintain accurate daily documentation of activities, fire drills, medications, and goal tracking. Transport individuals to community activities and medical appointments, or run errands as needed.
