Job Summary
Perform personal care tasks including assistance with basic hygiene, grooming, feeding, and ambulation. Implement Individual Service Plans and Behavior Support Plans while managing client behaviors through prevention or de-escalation. Encourage self-help activities and foster positive relationships between individuals served and their support circles. Keep accurate daily documentation of activities, medications, goal tracking, and safety drills. Transport individuals to community activities and medical appointments, or run errands for clients. Maintain a valid driver's license and auto insurance to support these duties.
Required Qualifications
