Conduct personal care tasks for clients, including assistance with hygiene, grooming, feeding, and ambulation, while monitoring medical needs. Implement Individual Service Plans and Behavior Support Plans to manage behaviors through prevention or de-escalation. Foster positive relationships with clients and their support circles while encouraging self-help activities. Transport individuals to community activities and medical appointments, and keep accurate daily documentation of activities, medications, and goal tracking. Maintain a valid driver's license and pass a background check to perform these duties in a home-like setting.
