Job Summary
Provide person-centered support to individuals with intellectual and developmental disabilities in residential settings, including direct personal assistance, health and wellness management, and meal preparation. Schedule and accompany clients to medical appointments, administer medications with AMAP certification, and manage their fiscal accounts while safeguarding funds. Maintain a clean, safe living environment, ensure proper staff-to-client ratios, and accurately document incidents, first aid treatments, and daily logs. Collaborate with team members and supervisors to implement individual treatment plans and promote emotional, physical, and personal well-being. Work flexible day, evening, overnight, and weekend shifts, maintaining alertness and adhering to all regulatory and compliance standards.
Required Qualifications
- Valid NY State Driver's License
- Driver's record considered acceptable by the agency and insurance carrier
- AMAP Certification
- At least one year of practical experience working with individuals with developmental disabilities
- High School Diploma or Equivalent
- Possess good verbal, reading, and written communication in English
- Possession of basic math skills
- Possession of basic computer skills
- Able and willing to successfully complete and apply training
- Possess communication skills that will effectively establish a collaborative relationship with individuals with intellectual/developmental disabilities
- Able to effectively teach, model and give support to individuals with intellectual/developmental disabilities
- Able to read, write, and interpret written documents
- Able to travel in all weather conditions throughout the counties served
- Clear background checks as defined by regulations and policies
- Able to maintain absolute confidentiality
- Able to demonstrate behaviors and attitudes which support agency mission, philosophy, and policies
- Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques
- Able to maintain Approved Medication Administration Personnel (AMAP) Certification
- Able to administer medications as written by providers
- Able to administer prescribed personal care by providers
- Able to complete 2nd med checks
- Able to schedule appointments as directed by the Residence Manager or Medical Liaison
- Able to attend medical appointments as directed by Residence Manager or Medical Liaison
- Able to work a combination of day, evening, overnight and weekend hours, as needed
- Able to remain alert and awake on all shifts
- Able to work cooperatively both independently and as a member of the Residential team
- Able to multi-task and prioritize duties
- Able to lift up to 25lbs
