Direct Support Professional-GH2

Conduct daily living support for individuals, including personal and health management, meal preparation, housekeeping, medication administration, and transportation to medical appointments. Develop Individual Support Plans, participate in planning team meetings, and complete required paperwork and logs accurately. Collaborate with licensing staff, doctors, and families while promoting client safety, inclusion, and community engagement. Assist with quality improvement projects and risk management goals to ensure high standards of care. Transport clients as required and maintain records of services and funds entrusted.
