Job Summary
Guide program participants by developing strategies, encouraging goal achievement, and coordinating community resources to support self-advocacy. Drive individuals to appointments and events while providing crisis intervention as needed to defuse situations and contact support systems. Maintain accurate documentation by managing electronic systems, submitting records timely, and conducting ongoing assessments to identify needs and capabilities. Build rapport through effective communication tailored to participants' styles and model core values including inspiring others and aligning with the community. Requires CPR/First Aid and Medication Management certification within six months, a valid driver's license, and reliable transportation.
Required Qualifications

- Must be CPR/First Aid and Medication Management certified within 6 months of starting employment
