Job Summary
Conduct direct support, positive direction, and assistance to CILA and ICF residents in accordance with program policies, ensuring safe and effective residential facilities. Implement and document individual goals and objectives while attending Individual Service Plans meetings to identify supports that improve quality of life. Assist individuals with activities of daily living, including household chores, recreational activities, grooming, and money management, while promoting independence. Prepare meals according to standardized recipes and assist with outings, shopping trips, and appointments by providing transportation and guidance. Administer medications, record vitals, and observe for medical concerns or accidents, reporting incidents immediately to the Resident Services Director or Nurse. Maintain confidentiality of personal information and comply with state and federal rules and regulations.
Required Qualifications
- High School Diploma or equivalent
- Age 18 or older
- Ability to read and comprehend English at an 8th grade level based on standardized testing
- Must be a certified DSP
- Certified in CPR/First Aid
- Certified for Medication Administration or enroll in the company provided courses, achieve passing scores and become certified
- Valid Illinois driver's license
- Able to pass a criminal background check
- Able to pass a drug screen
- Able to lift at least 50lbs
