Job Summary
Assist individuals with activities of daily living including personal hygiene, bathing, feeding, grooming, transferring, and meal preparation. Provide companionship, mentorship, and redirecting behaviors while administering medication and conducting routine wellness checks during overnight hours. Transport residents to appointments and community activities using a wheelchair van, and maintain accurate documentation of Individual Support Plans, daily logs, and behavioral data. Uphold individual rights, dignity, and confidentiality while ensuring a clean, safe, and supportive residential program environment.
Required Qualifications
- High school diploma or GED equivalent
- Ability to read, write, and comprehend English
- Experience in providing training, case management and appropriate service delivery to adults with intellectual and developmental disabilities living in the community
- MAP (Medication Administration Program) Certification
- CPR/First Aid certification
- Valid Driver's License in good standing
- Minimum of one year of driving experience
- Ability to lift heavy items
- Ability to drive wheelchair accessible company vans
- Full-time or part-time schedule availability (Monday, Wednesday, Thursday, Saturday & Sunday 3:00pm - 11:00pm or Saturday & Sunday 9:00am - 11:00pm or Saturday & Sunday 9:00am - 3:00pm)
- Union membership to SEIU Local 509
