Job Summary
Conduct routine health and wellness checks, including intake/output tracking and hydration monitoring, while assisting participants with mobility, transfers, and personal care needs. Transport approximately six to eight participants daily to and from the day program and into the community using an OPARC vehicle, adhering to all driver training guidelines and safety protocols. Maintain visual supervision during on-site and off-site activities, coordinate Individual Service Plans, and complete comprehensive daily documentation including incident reports and attendance logs. Report unusual health lapses, behavioral interventions, and environmental safety hazards immediately to management. Work Monday through Friday from 8:00 am to 3:30 pm with varied hours based on program needs.
Required Qualifications
- At least 18 years of age
- High school diploma or GED equivalent
- Valid California driver's license
- Clean driving record
- Three (3) years of driving experience
- Proof of current vehicle insurance
- Ability to pass a Department of Justice (DOJ) background check and fingerprint clearance
- Ability to complete TB test
- Ability to provide proof of required immunizations
- CPR certification
- First Aid certification
- Crisis Intervention (e.g., CPI) certification
- Ability to obtain CPR, First Aid, and Crisis Intervention certifications upon hire
- Ability to work in outdoor environments
- Ability to work in indoor warehouse-type settings
- Ability to lift up to 25 pounds by self
- Ability to lift up to 150 pounds with one other person
- Ability to push a wheelchair with a seated participant up to one (1) mile
- Ability to stand, walk, sit, push, bend, and pull
- Ability to squat
- Ability to perform repetitive motion
- Ability to use chemicals in accordance with manufacturer's labeled instructions
- Willingness to support individuals during community outings, job coaching, and/or volunteer activities
- Ability to work in both indoor and community-based environments

- Experience in a nursing role
- Experience in a behavioral role
- Experience in a human services role
- Bilingual skills (Spanish, ASL, etc.)
