Direct Support Professional Driver

Safely drive a van to transport program participants with developmental disabilities during scheduled hours, picking them up and dropping them off at the start and end of the day. Target person-centered goals to facilitate independence in community settings, including socialization, academics, and activities of daily living. Complete required documentation for billing, including daily checklists, weekly summaries, monthly summaries, timesheets, and billing forms using the MediSked system. Advocate for individuals served to encourage self-advocacy and decision-making skills while maintaining a professional relationship with co-workers and representatives from other agencies. Attend meetings such as Life Plan reviews and staff meetings upon request, and communicate any issues or concerns to other service providers for fluid coordination. Participate in all training assigned by the program supervisor or directors and follow incident reporting procedures in the event of a reportable incident.
