Job Summary
Assist people with disabilities in community-based settings by supporting transportation, appointments, personal care, and routines while following individualized care plans and safety protocols. Build meaningful relationships through respectful, person-centered support that encourages confidence and positive self-esteem. Collaborate with families, healthcare providers, and case managers to ensure services remain coordinated and responsive to each person's unique needs. Advocate for the rights, choices, and goals of individuals you support. Work Monday through Friday, 8:00 AM to 4:00 PM, for 37.5 hours per week, with occasional evenings or weekends as needed.
Required Qualifications
- High School diploma or equivalent
- Valid Driver's License
- Minimally one-year experience working with people with developmental disabilities
- Associate degree in human services or related field
