Direct Support Professional - Day & Residential Programs

Job Summary

Provide direct support to individuals in residential, day, and community settings by assisting with activities of daily living, personal care, medication administration, and skill development. Implement individualized plans, facilitate meaningful community participation, and transport individuals to appointments and recreational activities. Maintain safe environments, respond to emergencies, and complete accurate documentation in electronic case records while adhering to Connecticut DDS regulations and HIPAA requirements. Collaborate with interdisciplinary teams, families, and guardians to promote independence and self-determination.

Required Qualifications

  • Maintain all required trainings, certifications, licenses, and qualifications necessary to perform the essential functions of the position
  • Ensure all credentials remain current in accordance with agency, Connecticut DDS, and regulatory requirements
  • Participate in required agency, Connecticut DDS, safety, compliance, and professional development trainings
  • Demonstrate a commitment to continuous learning and quality service
  • Provide support and coverage across programs, locations, shifts, and service settings as assigned
  • Perform additional duties, assignments, and projects as requested to support program operations and the needs of individuals served
  • Maintain a safe, clean, organized, and welcoming residential environment
  • Maintain professional, respectful, and timely communication with individuals served, coworkers, families, guardians, community partners, and other stakeholders
  • Preserve confidentiality
