Job Summary
Assist consumers with developmental disabilities in skill development across independent living, adaptive behaviors, socialization, and activities of daily living. Supervise individuals in the community while reinforcing group session skills and ensuring safety. Lead group sessions to facilitate discussions that build community-useable skills. Assess needs of lower-functioning or non-verbal consumers through observation and evaluation to develop valued outcomes. Provide annual and semi-annual progress reports and maintain weekly documentation of outcomes per the Day Habilitation Plan. Ensure compliance with ISP and Day Habilitation Plans. Requires AMAP certification, SCIP-R training, and a valid NYS driver's license.
Required Qualifications
- High School Diploma or GED
- At least one year experience providing direct care services to adults with a developmental disability
- AMAP certified
- SCIP-R trained
- CPR & First Aid Trained
- Valid NYS Driver's license
- Must be able to pass insurance criteria
- Proof of vaccination against the COVID-19 virus
