Job Summary
Provide training and support in Activities of Daily Living to individuals residing in their own homes, implementing goals from Individual Service Plans. Deliver onsite and community-based assistance covering health monitoring, personal hygiene, domestic living skills, money management, meal preparation, socialization, recreation, safety education, transportation, and behavioral interventions. Maintain confidentiality, manage emergencies, and transport individuals using agency vehicles while adhering to all safety guidelines. Communicate individual needs verbally and in writing, document daily notes and reports, and participate in team meetings to discuss consumer programming and implementation of individualized goals.
Required Qualifications
- High School Diploma or equivalent
- Valid New York State driver's license with a clean driving record
- Trained in CPR/First Aid and maintained according to Red Cross standards
- Clearance from Office of Children and Family Services (OCFS) which includes NYS Department of Criminal Justice Services, Federal Bureau of Investigation, Statewide Central Register Database Check and NYS Justice Center
- Ability to assist with some personal care/toileting
- Ability to lift over 75 pounds
- Ability to react quickly, which may include but is not limited to stooping, bending, sitting, standing, stretching, walking and running
- May be exposed to various weather conditions
- May need to respond to a behavior intervention with an individual in the means taught to them in Strategies for Crisis Intervention and Prevention
- May be required to travel to various job sites or locations as part of working with individuals
- Evening and weekend work maybe required as job duties demand
- Monday through Friday during program hours
- 35 hours per week
- Full Time 12 Month
