Direct Support Professional - Community Support Facilitator (Part-Time)

Provide support to clients in their home and community, including assistance with personal hygiene if warranted. Assist with development and implementation of annual Personal Support Plan and Supported Living Plan goals. Participate in building relationships with family, friends, and neighbors to expand clients' participation in integrated community activities. Complete required documentation, progress notes, and medical appointment tracking while maintaining communication with supervisors and planning teams. Notify supervisors immediately of schedule changes or health and safety incidents. Drive your personal vehicle to assist clients with their goals, complying with federal, state, and local reporting requirements. Respond to supervisor and team communications within 24 hours, with urgent calls requiring immediate response.
