Job Summary
Provide support to clients in their home and community, including assistance with personal hygiene, cooking, cleaning, and driving them to integrated community activities. Assist with the development and implementation of annual Personal Support Plans and Supported Living Plan goals. Coordinate and monitor services such as MTS, Medi-Cal, and Social Security while maintaining communication with the supervisor and planning team. Complete required documentation, progress notes, and medical appointment tracking according to established deadlines. Attend mandatory meetings, trainings, and certification courses including CPR, First Aid, and AED. Enter timesheets, mileage reimbursements, and cell phone reimbursements into the time-keeping system. Notify the supervisor immediately of changes to the client's schedule or health and safety incidents. Respond to supervisor and planning team communications promptly, with urgent calls requiring immediate attention.
Required Qualifications
- High School Diploma/GED
- Valid driver license
- Annual vehicle inspection
- Vehicle insurance
- Vehicle registration
- 3 years driving experience
Desired Qualifications
- 1 year working in social services, education, or with the developmentally disabled population
