Job Summary
Support individuals with disabilities in personal care, daily routines, and community integration by assisting with activities of daily living, cooking meals, and maintaining a clean home environment. Transport supported persons to medical appointments, job sites, and community events using agency or personal vehicles while ensuring safe travel and resource management. Document shift notes in electronic records, oversee medication storage, and maintain confidentiality throughout all interactions with clients, families, and vendors. Work primarily in home and community settings, adhering to Support Solutions' philosophy of empowering self-directed lives.
Required Qualifications
- 18 years of age
- able to read and write effectively
- high school diploma or GED
- valid drivers license
- acceptable driving record
- meet state and local requirements regarding criminal background check
- meet state and local requirements regarding sexual offender check
- meet state and local requirements regarding abuse registry
- meet state and local requirements regarding drug and felony offender listings
- meet all training requirements
- keep training current
- provide medical records to comply with regulations required by a governing entity and Support Solutions policy
- TB Skin tests
- HEP B vaccinations
- Flu vaccinations
- full range of body movements including the use of his/her hands, feet, and limbs
- ability to bend the body, to reach for objects, and to crouch/stoop
- ability to climb when needed
- command of all five senses: sight, hearing, touch, smell, and taste
- close vision
- distant vision
- depth perception
- ability to talk and listen for prolonged periods both on the telephone and face to face
- ability to perform heavy lifting over 50 lbs
- Valid state issued Driver License
- valid Medication Certification
- valid CPR/First Aid Certification
- valid CPI (Crisis Prevention Institute) training
