Job Summary
Direct Support Professional (DSP) – Columbus, IN. Provide 1:1 care to individuals with intellectual or developmental disabilities at home and in the community, supporting an individual on Monday evenings in a part-time role. Responsibilities include assisting with self-care, ADLs, mobility, and activities to promote independence and community participation. Duties require documenting work records via AccelTrax with mandatory Electronic Visit Verification, producing shift narrative notes, and completing progress notes with goals. Training is provided on Indiana Home and Community Support Professional standards, Behavior Support Plans, Risk Plans, Medication Administration, and Incident Reporting. Familiarity with 1:1 support, home and community-based services, and maintaining records of activity and time is expected.
Required Qualifications
- Familiarity with or training in Indiana Home and Community Support Professional standards
- Experience documenting work records via AccelTrax and using Electronic Visit Verification (EVV)
- Ability to complete narrative notes for each shift including location and duration of activities
- Ability to complete progress notes and select goals worked on
- Training on Behavior Support Plans, Risk Plans, and Medication Administration; and completion of Incident Reporting as needed
- Experience providing 1:1 care to individuals with intellectual or developmental disabilities
- Comfort assisting with self-care, ADLs, mobility, and community participation
