Job Summary
Provide ongoing guidance and support to individuals with intellectual and other disabilities, focusing on self-care routines, health, safety, household chores, and socialization. Assist with medication administration, meal preparation, and transportation to medical appointments and community outings. Implement and document treatment plans, including residential habilitation goals and communication logs, while adhering to agency policies and scheduling requirements. Mentor new staff by demonstrating supportive routines and reviewing training manuals. Participate in interdisciplinary team meetings to discuss individual progress and maintain certification and training expectations.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must be able to meet the physical demands of the position

- High School Diploma or equivalent
