Job Summary
Provide ongoing guidance and support to individuals with intellectual and other disabilities, ensuring active participation in self-care routines, health, safety, household chores, and socialization. Assist with medication administration, meal preparation including diet adherence and cooking skills, and transportation for medical appointments and recreational outings. Implement and document treatment plans, residential habilitation goals, and communication logs while maintaining a safe and clean home environment. Mentor new Direct Support Staff by demonstrating supportive routines and reviewing training manuals. Participate in inter-disciplinary team meetings and house meetings to discuss progress and gather ideas. Adhere to overtime, scheduling, and minimum staffing policies. This role requires regular unsupervised physical contact, a valid NYS driver's license, and successful completion of a fingerprint/criminal background check.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
