Direct Support Professional (CLS)

Provide ongoing guidance and support to individuals with intellectual and other disabilities, ensuring active participation in self-care routines, health, safety, household chores, and socialization. Assist with medication administration, meal preparation, and transportation to medical appointments, shopping, and recreational outings. Maintain a safe and clean home while documenting treatment plans, residential habilitation goals, and communication logs. Mentor new Direct Support Staff by demonstrating supportive routines and reviewing training manuals. Serve as a member of the inter-disciplinary team by attending meetings to discuss progress and gather information. Adhere to overtime, scheduling, and minimum staffing policies. This role requires regular unsupervised physical contact, a valid NYS driver's license, and the ability to lift and transfer individuals. Successful completion of a fingerprint/criminal background check and Personal Outcome Measures training is required prior to evaluation.
