Job Summary
Provide ongoing guidance and support to individuals with intellectual and other disabilities, ensuring active participation in self-care routines, health, safety, household chores, and socialization. Assist individuals in preparing meals, administering medication per policy, and maintaining a safe, clean home. Implement and document treatment plans, attend inter-disciplinary meetings, and provide transportation for medical appointments and recreational outings. Mentor new staff by demonstrating supportive routines and reviewing training manuals. This role requires regular unsupervised physical contact, a valid NYS driver's license, and the ability to lift and transfer individuals. Successful completion of a fingerprint/criminal background check is mandatory.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
