Job Summary
Provide ongoing guidance and support to individuals with disabilities in self-care routines, health, safety, household chores, socialization, and overall wellbeing. Assist with meal preparation, medication administration, and transportation for medical appointments, shopping, and recreational outings. Implement and document treatment plans, including residential habilitation goals and communication logs, while adhering to agency policies and inter-disciplinary team meetings. Mentor new staff by demonstrating supportive routines and reviewing training manuals. Maintain a safe and clean home environment and participate in house meetings to ensure certification and training expectations are met.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
