Job Summary
Conduct ongoing guidance and support for individuals with intellectual and other disabilities, focusing on self-care routines, health, safety, household chores, socialization, and overall wellbeing. Ensure a safe and clean home by assisting with and participating in household tasks, while supporting meal preparation through diet adherence and cooking skills training. Administer medication according to AMAP policy and implement all treatment plans, including residential habilitation goals and medical case notes. Provide transportation for medical appointments, shopping, and recreational outings, and mentor new staff by reviewing training manuals. Participate in inter-disciplinary team meetings and house meetings to discuss progress and goal achievements. Maintain all certification and training expectations, including Personal Outcome Measures (POM), and adhere to overtime and staffing policies. This role requires regular unsupervised physical contact, a valid NYS driver's license, and the ability to lift and transfer individuals.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
