Direct Support Professional (CLS)

Job Summary
Provide ongoing guidance and support to individuals with intellectual and other disabilities, focusing on self-care routines, health, safety, household chores, socialization, and overall wellbeing. Assist with medication administration, meal preparation, and transportation to medical appointments, shopping, and recreational outings. Implement and document treatment plans, including residential habilitation goals and communication logs, while adhering to agency policies and minimum staffing requirements. Mentor new staff through supportive routines and training reviews. Participate in inter-disciplinary team meetings to discuss individual progress and maintain certification and training expectations. This role involves regular and substantial unsupervised physical contact, requiring a valid NYS driver's license and successful completion of a criminal background check.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
